02What it is

A typical menstrual cycle is around 28 days with 4–5 days of bleeding. Periods are considered irregular when bleeding comes sooner than 21 days, later than 35 days, or when consecutive periods are missed. Irregular cycles are common and often treatable once the underlying cause is understood.

03Who it's for

This service is for women whose periods last longer than a week, who soak through a sanitary pad in an hour or bleed more heavily than usual, whose cycles fall outside 21–35 days, who miss three or more periods in a row, or who have spotting between periods. Periods with severe pain, cramping, nausea or vomiting are also a reason to be seen.

04How we diagnose

Assessment starts with a review of your medical history and a pelvic examination, and it helps to track your flow, duration, cramping and clotting beforehand. Depending on findings, we may use a pelvic ultrasound to look for fibroids, polyps or cysts, an endometrial biopsy for tissue analysis, or a hysteroscopy to view inside the uterus directly.

05How we treat

Treatment is chosen to fit your health, your wish for future pregnancy and how severe the symptoms are. Options range from medication — such as hormonal contraception, tranexamic acid for heavy bleeding, pain relief, hormone therapy, antibiotics for infection, or GnRH agonists — to procedures where needed, including endometrial ablation, uterine artery embolization, myomectomy to remove fibroids, or, only as a last resort, hysterectomy.
